We believe in a holistic, needs-driven approach that listens to and responds to each young person’s unique needs. Our services are flexible and innovative, always centered around collaboration with young people, their families, and other relevant agencies. At Floridus Care, we treat every young person with respect, and we take pride in celebrating the diversity of our residents.
Client Group
We primarily support young people and our services extend to those with learning difficulties or who face challenges in living independently after 18. This includes:
- Young people whose care plans have been disrupted by multiple placement moves.
- Those dealing with mental health issues who need help managing their own accommodation.
- Individuals with learning difficulties who require ongoing support to live independently.
- Young people with gang affiliations or at risk of CSE who need safeguarding and additional support beyond age 16.
- Those waiting for permanent accommodations that aren’t ready yet and need temporary support.
We understand that these young people often require varying levels of assistance, and we’re here to help them navigate these transitions with care and understanding.
Our approach involves timely interventions and tailored support packages that align with each young person’s pathway plan and specific needs. We focus on building positive, trusting relationships, which not only helps us better understand and meet their needs but also makes it easier for them to share their concerns. Our experienced staff are always ready to provide the right level of support, ensuring every young person feels heard and cared for.
Our Services
we provide a comprehensive range of support to ensure our young people thrive. Our services include:
- Collaborating with Social Workers and young people to complete pathway plans, CSE risk assessments, and safety plans.
- Registering young people with local opticians, dentists, and GPs.
- Accompanying young people to routine health appointments, dental check-ups, and optician assessments.
- Supporting young people in attending sexual health clinics and CAMHS meetings, in consultation with them and their Social Workers.
- Discussing health and well-being during keyworking sessions, with a focus on the negative effects of drugs, substance misuse, and unhealthy associations.
- Providing weekly and monthly placement reports upon request by the Local Authority.
- Conducting Placement Planning Meetings (PPM) with young people and their Social Workers within five days of placement with Pathfinders.
- Preparing young people for independent living and greater self-sufficiency.
- Engaging young people in direct work and creating weekly activity programs.
- Registering young people with local gyms and encouraging them to maintain a healthy lifestyle through exercise.
- Partnering with agencies to safeguard young people from harm or abuse.
- Connecting young people and their families with relevant services, such as counseling and mediation, to meet their needs.
- Assisting young people with housing applications and bidding for permanent accommodation online.
- Offering up to 5 hours per week of one-on-one keyworking support sessions.
- Providing 24-hour staff support, including sleep-in/waking shifts, and out-of-hours contact with an on-call manager.
- Ensuring quality-assured assessments overseen by a Qualified Social Work Manager.
- Offering practical support to young people transitioning to permanent accommodation after funding approval, including help with setting up their new homes.
- Assisting with the home setup process—searching for and viewing accommodations, shopping for furniture and appliances, setting up utilities, and completing benefit forms.
We’re committed to making sure our young people are fully supported every step of the way, ensuring they have the tools and guidance needed to succeed in their journey to independence.
Client Relationships
Every young person is paired with a dedicated key worker who collaborates closely with Social Workers and other agencies to meet their unique needs. Our skilled key workers take a holistic approach, ensuring that our young people are supported in all aspects of their lives and engaged in meaningful activities aligned with their care and placement plans.
Our key workers are trained to conduct thorough assessments and are well-versed in partnering with Social Workers to complete pathway plans, Child and Family Assessments, and Risk Assessments.
They also provide detailed weekly and monthly placement reports to Social Workers as needed.
When required, our key workers can also act as Appropriate Adults for young people in police custody, with approval and consultation from Social Workers, if this is part of the placement package and funding has been secured.
Young people participate in weekly keyworking sessions based on their needs and availability, ensuring they have consistent support and guidance. Our standard service package includes five hours of keyworking support each week, providing a reliable point of contact for ongoing care. If additional keyworking support is needed, we’re always ready to adapt.
Accommodation
We provide safe, supportive, and comfortable accommodation tailored to the needs of each young person. Our homes are more than just places to stay—they are environments where young people can grow, learn, and prepare for independent living.
Each accommodation is carefully selected to ensure it meets high standards of safety and comfort. We create a welcoming atmosphere that encourages young people to feel at home while developing the skills they need for the future.
Our team is dedicated to maintaining these spaces, ensuring they are well-kept and conducive to a positive living experience. We also offer practical support, helping young people personalise their spaces, manage household tasks, and gain the confidence to live independently.
